Fairbanks Youth Soccer Association

 This form is set up for multiple programs:

• Youth recreational leagues for grades K-HS and select (advanced rec) divisions
• Youth soccer camps & clinics, offered through FYSA and UK International Soccer
• PreK soccer mini-seasons for youth ages 3-4
• I Can Play Soccer 2 (ICPS2) - toddler sessions for 2 year olds
• Adult soccer leagues for those ages 14 and older

HOMESCHOOL FAMILIES: If you are wanting FYSA to direct-bill your homeschool program, please contact us to BEFORE completing this registration to receive a code that will allow the form to be completed without payment. We will need some additional information in order to invoice your program to utilize allotment funds.
